New Patient Registration

Before completing the on-line registration documents, or collecting the relevant forms from Reception please telephone the surgery to see if we are taking patients this month.  The Health Centre has three GP Surgeries and between us we have a rota system for taking on new patients.

It is advisable that all patients wishing to register with this practice check that they are  living within our catchment area (see below). An appointment should be made for a new patient check with the nurse. Patients will become a patient of the practice, although they have a right to express a preference of practitioner. The practice will endeavour to comply with any request but it might not be possible, in which case an explanation would be offered. Our Registration Clerk is available Monday to Friday from 9 am to 11.30 am if you have any queries.
